The kmail composer does highlight spelling errors but also highlights words that you have added to the dictionary using the kmail composer spell checking tool.  When words are added to the dictionary then they should not be highlighted any more.  

If it helps the highlighting of web form text boxes (Konqueror 3.3.2 Debian testing on linux) is correct against the custom added words.  Since adding the words they are not highlighted in web form text boxes as they are in the kmail composer.

I wonder if restarting KMail helps. If so perhaps the code to 
re-spellcheck the mail when the composer re-gains focus has been 
inadvertently disabled. If not unfortunately I don't have a 
suggestion.
