This is how Acreread renders the file
It seems it's a review, you can right click on it and remove it, but i'm not sure the review contents are correct Same problem. I added it a hint as "textfield" with the graphic tools in Adobe Acrobat 7.0. The pdf and screenshots with okular, evince and xpdf are added. Created attachment 35694 [details]

scanned with a scanner in a library and an added textfield by Acrobat 7.0 from the graphics toolbox.
It seems that kpdf-0.5.10 also renders the files in this bug properly.
