It seems it's a review, you can right click on it and remove it, but i'm not sure the review contents are correct
Same problem. I added it a hint as "textfield" with the graphic tools in Adobe Acrobat 7.0. The pdf and screenshots with okular, evince and xpdf are added.

It seems that kpdf-0.5.10 also renders the files in this bug properly.

This was actually a poppler bug that's been fixed in poppler 0.20. A workaround for older poppler versions is present since okular 0.15, ie KDE 4.9
