Screensaver starts automatically initially but after a few times of use it doesn't do that.

I have these settings:

[x] Start automatically after 5 mins
[x] Require password to stop after 60 seconds
[x] Make aware of power management

I have the same issue as well, under SuSE 9.2 and when I used their rpms to upgrade to KDE 3.3 under SuSE 9.1. Version 3.3 under 9.2.  
My current settings are 10 min for screen saver, 60 sec for password required, power management enabled. All DPMS options are set to 20 min. I can see just from this file that the gui front end is not setting the DPMS right. Also, if I set the screen saver to come on after a minute, it will.

Same problem for me. This started happening after I upgraded X11 and gentoo switched me from XFree86 to Xorg. Is that a common thread? For what it is worth, here is my xset -q:
