When clicking on a menu item, the menu should never appear beneath the pointer (i.e. with an item already selected), because when the mouse is released, the selected item will be executed, before the user has had a chance to see what they've selected.

This happens most often on the bookmarks menu when it is overly full, and 'Add Bookmark' is executed every time I click on the Bookmarks menu unless I remember to click and hold.

The other place it happens is on context menus. Most of the time a context menu appears with its corner at the mouse position, but every so often, for no apparent reason, one will appear beneath the pointer instead - most often on the 'Quit' entry of a system tray icon.
