SUMMARY
Can reproduce on both Arch Linux and neon unstable.

STEPS TO REPRODUCE
1. make sure the task manager is set to open a new window on middle-click
2. activate "Vaults" applet in the system tray and click on its "+ Create a new vault..." button
3. middle-click on "Create a new vault" item in the task manager

OBSERVED RESULT
the notification seen in the attached screenshot appears

EXPECTED RESULT
no notification after the provided steps

I'm not able to reproduce this in Plasma built from git-master on Solus or with Neon Testing. Middle clicking the "Create a new vault" button does nothing.

Can you share a screenshot of Settings - Notifications - System Notifications? You may have a notification type turned on that I don't. 
Also, a recording of reproducing the bug would be helpful.

Thanks for sharing the recording. There weren't any significant differences in notification settings on my systems.
The recording did highlight that the Text and icons task manager is being used, and that clicked on specifically

I was able to reproduce the bug with the Icons-Only task manager, middle clicking on the icon for the Create a New Vault dialog as well
