SUMMARY
When configuring the toolbars, the content of the Main Toolbar is shown four times. The toolbar itself shows it only once.

STEPS TO REPRODUCE
1. Go to Configure Toolbars
2. Select Main Toolbar
3. Observe Current Actions

OBSERVED RESULT
The default items show up four times

EXPECTED RESULT
The default items show up only once each

I cannot reproduce on 25.08.3. Flatpak toolbar config is located in :

~/.var/app/org.kde.kdenlive/data/kxmlgui5/kdenlive/kdenliveui.rc

For some reason, your file was somehow corrupted, no idea why. If you delete this config file (or do reset config in Kdenlive > Help), it will come back to normal.

Do you remember doing some specific things to the toolbars config ? Unless we find a way to reproduce, it will be hard to fix this one

There were indeed four instances for each of the Main Toolbar items in that config file. But deleting the extra ones did not fix it: There was still a second copy of the entries. I ended up deleting the extra ones, and now there is only the one set in the config file and in the Configure Toolbars dialog window.
