Thanks for reporting this. Your Okular version is 2 years old, and many forms-related bugs have been fixed in the meantime. Can you try a newer Okular version and see if the bug persists? I tried downloading the most recent source, but I'm not set up to build using clang, and I don't have time at the moment to set things up. I have managed to fill in the form through other means. If I get some spare time I'll look further into it. I haven't time to build a more current version right now. Okular is started from command line. Okular window offers a "show form" button. When I click on the button the fillable fields appear. I can select a field and place the cursor in it. Okular crashes imediately when I attempt to begin typing into the field.
