SUMMARY
From https://flatpak.github.io/xdg-desktop-portal/docs/doc-org.freedesktop.impl.portal.Account.html, I get an impression that the response should contain an "image" field regardless.

STEPS TO REPRODUCE
1. Install and launch https://flathub.org/apps/com.belmoussaoui.ashpd.demo
2. On the "Account" page, enter a reason and click Request
3. Accept the portal dialog

OBSERVED RESULT
"Request to fetch user information failed". From the demo's logs:
"ERROR ashpd_demo::portals::desktop::account: Failed to retrieve user information: ZBus Error: missing field `image`"

EXPECTED RESULT
Account information is fetched successfully

Tested with GNOME 47, works there
