Does the issue go away if you switch to the default icon theme or Plasma theme?

The Plasma theme I'm using is the default Breeze Dark, but even after switching icon theme it doesn't go away.

There's nothing nonstandard about the buttons or the layout they live in. I'm at a loss to explain how this could happen, and I can't reproduce it myself.

Does it happen in a new clean user account?

I tried reproducing it a few times after but to no avail, and it never happened again, so I assume it was some strange, one time issue. Thank you for your time to look into it.
