Wrong directory in a tab title

When the tab title is set to a default title, "%d : %n", at first it follows the current directory when switching directories but at some point it stops updating. The new tab is then opened in the wrong directory, not the current directory of the original tab.

I attach the screen showing the problem: the left tab's current directory is "~/Documents/PhD Studies" but the tab title incorrectly says "Universities". The right tab was open in the "~/Documents/Universities" directory, not in the left tab's current directory.

Steps to reproduce:
1. Open Konsole and set the tab title to "%d : %n".
2. Switch directories a few times until the tab title stalls.

Observed result: tab title doesn't change with the tab's directory changes.

Expected result: tab title follows the tab's current directory.

Can confirm this behaviour in Arch Linux with the latest packages installed and fish shell in use.

Can also note that starting Midnight Commander updates the tab to the current folder, but simple cd doesn't.
