STEPS TO REPRODUCE
1. Open link1, play video and make sure video volume is at 100%. 
2. In the application volume panel, Firefox is shown to be playing at 100%.
3. Open link2, play video and make sure video volume is at 100%.
4. In the application volume panel, Firefox is shown to be playing at 48% (or at least not 100% like it's supposed to).

OBSERVED RESULT
Widget volume slider doesn't match the media actual volume level.

While doing more research on the behavior I noticed that the application slider in the applet was changing every single time I clicked another video on YouTube.

e.g.,
Watch video1 with stream slider is at 100%, applet slider is at 58%
Click video2 on the same page, stream slider is at 100%, applet slider is now at 67%.
Click video3 on the same page, stream slider is at 100%, applet slider is now at 49%.

This slider changes when a new video is loaded from the same page, in sequence. The stream slider is at 100% but the one from the screenshot varies from video to video. If a video sets the applet volume at 80%, it will also be 80% for that one. Same for other videos. I have a feeling that it's interpreting a particular value from the video.

When it happens, the only way to truly have the volume at 100% is to max the applet slider since the stream slider is already at 100%.

Looks to me like a firefox bug. Chromium doesn't exhibit this behavior. The volume also gets displayed correctly, according to pactl the stream is not at 100%.
