SUMMARY
My shortcuts: Meta+Tab for Walk through windows, Meta+` for Walk through windows of current application. 
I selected "Only one window per application" in System Settings -> Window Management -> Task Switcher
See below. 

STEPS TO REPRODUCE
1. Have multiple windows of same applications (e.g. 3 Alacritty windows)
2. Meta+Tab (don't release Meta key), window switcher shows up, and there's 1 Alacritty window (expected)
3. Then hit ` key (keep holding Meta key) to "walk through windows of Alacritty"

OBSERVED RESULT
There should be 3 Alacritty window thumbnails in window switcher

EXPECTED RESULT
There's only 2 thumbnails, the third thumbnail slot is present but the thumbnail is missing

ADDITIONAL INFORMATION
Honestly after Plasma 6 update there's several new bugs and performance decreases. Sorry but I have to complain, the experience has been bad.

Cannot reproduce on Wayland with current git master with any app I try it with, including Alacritty.

Does it reproduce for you with all apps tested, or only Alacritty?

Yeah, with konsole, alacritty and Dolphin, at least. Haven't tested with other apps yet.

Adding new info here, this issue happens to all other task switchers, like Compact, Sidebar, Large Icons e.t.c..
And I'm using Xorg, and I cannot test it on Wayland due to old graphic card. @Nate Graham knows about it and uses Wayland, so I guess it's probably an Xorg only bug?
