SUMMARY
The "Code Action" context menu item is no longer at the top of the context menu, which makes it uncomfortable to use and not possible to use it mouse only.

STEPS TO REPRODUCE
1. Open a file in Kate with an LSP
2. Right-click on a highlighted actionable item

OBSERVED RESULT
The "Code Action" context menu is in the middle

EXPECTED RESULT
The "Code Action" context menu item is at the top
