STEPS TO REPRODUCE
1. Clear kscreen config
2. Plug in external monitor via HDMI

OBSERVED RESULT
Monitor works, but it's using 1024x768 resolution. This is not only way below what the monitor is capable (3840x2160) but also is 4:3 instead of the monitor's native 16:9. This results in a very poor and distorted image. 

EXPECTED RESULT
An appropriate resolution is chosen, i.e. the highest available or at least one that matches the monitor's aspect ratio

Does this also happen if you disable KScreen? Because I ran your modelist through the algorithm used in KWin and the 2560x1440@59.95 mode gets chosen.

Same thing after disabling the kscreen kded module
