1.  Unsure, it appears to be random

OBSERVED RESULT
At times, changes are not detected (File/Save remains ghosted) and ctrl+s does not update the file.

EXPECTED RESULT
Changes to be saved with ctrl+s

I remember this happening at least since 0.7.0 on openSUSE Leap 15.4 and 15.3 as well.
