STEPS TO REPRODUCE
1. Create new shortcut by pressing 'Add Command...' , save, apply
2. Edit created command by pressing the pencil icon, save or cancel to exit the dialog
3. Create new shortcut by pressing 'Add Command...'

OBSERVED RESULT
Instead of having a new create shortcut window appear, an edit window for the last shortcut appears. When editing in the text field the dialog sometimes randomly closes. When saving, the previous shortcut gets edited 

EXPECTED RESULT
A new shortcut should be created from a fresh (empty) create dialog.

This is already fixed in Plasma 6.
