SUMMARY

okular doesn't show checked checkboxes in this document:

STEPS TO REPRODUCE
1. open the above doc
2. click "Show Forms"
3. check one of the checkboxes
4. click "Show Forms" again
5. observe that this checkbox isn't checked in okular

OBSERVED RESULT
Checkboxes aren't checked in okular (on screen) when they are checked in the document.
The resulting PDF document actually has checkboxes checked.
