SUMMARY
I'm using KMix on Debian bookworm (packaged KMix 4:22.12.3-1).
I use LXDE

STEPS TO REPRODUCE
1. Run KMix
2. Restore its window
3. Look at the taskbar or main application icon (top-left of the window)

OBSERVED RESULT
LXDE placeholder icon for applications without an icon

EXPECTED RESULT
pinux:/usr/share/icons/hicolor/16x16/actions/kmix.png

ADDITIONAL INFORMATION
I have the same problem with KOrganizer and akregator. Other applications such as kcalc, kwrite and kate show the correct icon.
I've checked and kcalc, kwrite and kate use "QApplication::setWindowIcon" or "app.setWindowIcon" with the icon to set the icon of the application.
