KMix vs plasma-pa tray applet after several bluetooth audio device reconnections

SUMMARY
Plasma tray's audio volume applet doesn't remove disconnected bluetooth devices from list, causing list trashing and unability to use volume keys to control volume

STEPS TO REPRODUCE
1. Pair with bluetooth audio device
2. Walk round to get several (dis-)connected messages
3. Check audio devices in plasma-pa and kmix on par
4. check volume up/down and (mic) mute buttons

OBSERVED RESULT
plasma-pa applet has a lot of dupes of same bluetooth audio device, KMix hasn't
volume buttons are doing strange glitchy things

EXPECTED RESULT
plasma-pa and KMix has the same devices list
volume buttons are working
