SUMMARY
***
I did a lot of scans in 300 DPI, produced images are about 27Mib, every now and when I scanned a page upside down, so I need to rotate them. Rotating changes the image size.
***

STEPS TO REPRODUCE
1. Open big image
2. Rotate left twice to achieve 180
3. Save

OBSERVED RESULT
Image is now 7.9Mib, down from 27Mib, resolution is the same

EXPECTED RESULT
Image should be the same size.

Where can I share an example file of this size?

Thank you for the bug report. Unfortunately we were not able to get to it yet. Can we ask you to please check if this is still an issue with Gwenview 25.08.0?
