I am using kghostview 0.12.  I live in a technologically backward country
that uses "letter" paper but kghostview's default paper size is "auto"
which seems to mean A4 unless the PostScript code specifies otherwise.
How can I configure or invoke kghostview to use "letter" size by default?

From what I read of the code and CVS logs this has been fixed since version 0.12 
of kghostview by a commit of wgreven to kgv_miniwidget.cpp (version 119). 
 
Now, kghostview gets its page size from the current locale as set in kde's control 
centre.
