I encountered this issue but not anymore, I believe it was a bug elsewhere that got fixed. Could you confirm the fix once you get KDE Frameworks 5.87 (it is due October 9th) Just retested with frameworks 5.87. Still no default button selected. I still can't reproduce it (with dolphin and kio from sources). It might be a dolphin issue then, or something more subtle. Even after a system reboot I still have the same issue.
