SUMMARY
I have been having this problem seemingly randomly, sometimes it happens and sometimes it doesn't, and I finally got a way to reproduce it. 

It has also been reported in the reddit community. However the author apparently didn't create an issue here.

STEPS TO REPRODUCE:
1. Disable the title bar in firefox (hamburger menu -> customization -> title bar checkbox, at the bottom of the window)
2. Create a kwin rule for the firefox window and under Appearance and fixes select "No title bar and frame". Set the Force option and check "No".
3. Open firefox

Although I tested in firefox, this worked in every GTK application I have installed.

This bug was reported against an outdated version of KWin. We have made many changes since the. 
If the issue persists in newer versions can you reopen the bug report updating the version number.
