SUMMARY
In 5.19 KDE version all works.

STEPS TO REPRODUCE
1. Plug headphones

OBSERVED RESULT
No sound, no sound output devices.

EXPECTED RESULT
Sound on, sound output devices not empty.

1. Unplug headphones

OBSERVED RESULT
Sound on, sound output devices not empty.

EXPECTED RESULT
No sound, no sound output devices.

Could be the same root cause as Bug 434225. Or it could be a bug in PulseAudio itself, such as https://gitlab.freedesktop.org/pulseaudio/pulseaudio/-/issues/939.

Since those commands help, it seems certain that this is a bug in PulseAudio itself, and not anything in KDE code. One of those two, perhaps.
