What I mean is,f you change your global audio,then there's a little window.
It would be cool if you could change where it spawns,how big it is,if it is horizontally or vertically orienatated,how it looks etc. in system settings.

That's the OSD. It's used for various status notifications, not just volume. It's configurable in the plasma theme, meaning you can change its characteristics by either getting a new Plasma theme, or editing your existing one. Being able to edit the OSD beyond that is something that's pretty much out of scope.

So would it be hard to implement a option to change the location were the osd gets spawned and which exact size it has?
And what is about if I want that there is a volumen or decibel number instead a big fat sound symbol?
Could it be possible to add a feature like that?

OSD positioning is governed by the Plasma theme.

Displaying the volume in decibels seems like unnecessary information to me, if it's even possible to get that level of information out of PulseAudio (no idea whether it can be done or not; I'm not a PulseAudio expert).
