You can sometimes get into a position where only two tiles are left on the board, stacked one on another. That's clearly unsolvable and you lose the game. 

However if the tiles are the same, the game should be won, not lost.

Can you point to some rule book where it actually says that is a win and not a lose?

I don't have a clue. Mahjongg is typically a multiplayer game, so I couldn't find anything relevant to this implementation. So maybe a configurable option? It's infuriating when it happens and doesn't make much sense since pieces are removed one by one.

The game is created in a way that it is always solvable. Finding that way is the (only) challenge.
