This looks like a packaging issue. LabPlot uses KDE's icons. Can you check whether breeze icon set is installed on your system?

Also, the current release is 2.6:

If possible, please give the new version a try.

Breeze icon theme is installed, but it doesn't solves the problem.

Can you try 2.6 or even better the development version if the problem still exists?

I read you problem description again. It was about icons in the menu and not in the toolbar. Icons in the toolbar are shown correctly according to your screenshot. For the menu titles there are no icons, only for different menu entries if you open such a menu. So, this is not a bug.
