SUMMARY
As we can see in the attached screenshot, "Press left and right buttons for middle click" is greyed out on X11.

STEPS TO REPRODUCE
1. start plasma X11
2. search for "touchpad" kcm in the apps launcher and open it
3. 

EXPECTED RESULT
"Press left and right buttons for middle click" option can be checked/unchecked

Does your touchpad have separated physical left and right-click buttons? Or is it all one big square pad?

It have separated physical left and right-click buttons.

This issue only affects standalone Touchpad KCM.
I can check/uncheck "Press left and right buttons for middle click" in system settings > input devices > touchpad.

Greyed out "Press left and right buttons for middle click" checkbox is expected because my device does not support such feature.
