It happened again when using the default theme and using the papirus icon theme. Did you observe the note regarding that theme? Adapta KDE - This is a port of the popular GTK theme Adapta for Plasma 5 desktop with a few additions and extras. Breeze engine not supported!!! I assumed since it's an integrated offer, it'd work out of the box. I little concerning that you're able to download/change to a non working theme.
