Dolphin cannot open audiocd even if Kb3 can (amarok also fails). There is an AudioCD icon but clicking on it does nothing.

When launched from a terminal, dolphin says:

couldn't create slave: "Unable to create io-slave:\nklauncher said: Unknown protocol 'audiocd'.\n"

If I type: audiocd:/?device=/dev/sr0 still nothing happens.

Just typing: audiocd:/
returns: Invalid Protocol

You need audiocd-kio >= 16.12

...And it's up to your distro's packagers to satisfy that dependency. Are you using an old version of Ubuntu?

OK, please report a bug on Launchpad indicating that the audiocd-kio package with a version greater than or equal to 16.12 is required as a dependency for Dolphin.

If that dependency cannot be satisfied because the version of Dolphin available in Ubuntu 16.04 is too old, than I'm afraid your options are limited to upgrading to a newer version of Ubuntu, using the KDE Backports PPA, or compiling Dolphin from source.
