Not sure if this is a plasma-pa bug or a pulseaudio bug. In plasma-pa -> Audio Volumue -> Advanced tab.... checking the "Automatically switch all running streams when a new output becomes available" causes PA to crash on startup. 

If the /etc/pulse/default.pa file is modified by commenting out following line:

  load-module module-switch-on-connect

PA will start as expected. Auto switching works great even when the above line in  default.pa line is commented out.

That's an issue in PulseAudio.

I filed the bug here but received no response:

Launchpad would be downstream, but the bug is upstream in PulseAudio.
