Two problems:

Start Konqeror with image loading disabled to reproduce them.

1) Images in the <input type="image"> element are not shown as outlines nor is alt text printed even if present

This does not show an outline of the image nor does adding an alt tag help.

2) load a page that contains images
rightclick on one of the image outlines select "view image" -->
a new window opens that a) does not
display the image and b) does not have the load images icon in the toolbar.
