There is a button located bottom right in notes widget that opens the settings dialog and is invisible with every plasma theme. See screenshot.

Try removing your icon caches in ~/.cache/ - namely files icon-cache.kcache, plasma_theme* and plasma-svgelements*.

Then log out and back in and then post here if that fixed it.
