Wrongly rendered file dialog
This is how the file dialog appeared after step 3. The window size is correctly restored, but the window contents don't fit into the window size.
An easy workaround is to grab the resize handle at the bottom right corner of the window border. The next resize event fixes the rendering glitch and resizes the window contents to the proper size.

Since this might be related to KWin or to the graphics stack in general, I'm also attaching the KWin support information for my system.
