Unfortunately, I do not know what causes this but I have experienced this several times. I try to refrain from reporting on issues I can't reproduce but this has happened way more than once and is definitely bothersome. If you know how to reproduce it, please tell. I'm currently on Fedora 22, update-testing repos enabled along with plasma-5-beta using Nvidia proprietary drivers 352.41 This happens to me as well quite often, even in KDE Plasma 5.5.4. I can reproduce this on my identical monitors just by turning the monitors off and then on again. (Hello, IT?) Or by coming out of sleep mode without first turning the monitors on. Definitely most annoying! Please attach the output of `kscreen-doctor -o` when the layout is set how you want, and then again after it gets messed up. Thanks! Messed up configuration after switching on one monitor too late: Symptom: Chrome moved from right display to one half on each screen. Kpatience moved from left to right. Orientation of screens not affected.
