I tried this and the only thing I can see is that the tooltip briefly (one frame?) disappears when hovering the next entry, but that's it.

Do we have a better explanation, screenshot or video of what happens?

Make sure you have both "Show Tooltips" and "Highlight Windows" enabled in the Task Manager, then hover various tasks. When the tooltip show/hide/move animations happen at the same time as window highlighting tweening, there's flickering and ghosting.

Yes, I had that and no, that simply didn't happen.
Sounds a bit like related to the blur cache, maybe animation time?

kcmshell5 kwineffects, filter for blur, configure it, uncheck the saving of interim results.

i have no problem with the tooltip, but the described behavior applies to the popup menu for choosing among grouped windows.

and the guess about the blur effect seems to be spot-on.
