I use konsole as my IDE.  I currently have four windows opened and about a dozen or so tabs on each window.  I generally use the keyboard to navigate between each window/tab.  Today, I clicked the minimize button on each of the tabs (attempting to get to the desktop to fix a application link).  When I pulled all of the windows back up, every window had been renamed to the default "shell %#".

This is not really what I would expect.  I have no way to know what I did to make this happen, no way to repeat this (I HOPE!!!), and no clue of any special events that may have caused it.

Which text was wrong? The text on the tabs or on the window title? Your bug report isn't really clear on this.
