When activating replay gain and fadeout the replay gain does not apply immediately. So if there are are two files with very different loudness I can hear the volume adjustment after about a second. Deactivating fadeout fixed this.

Reproducible: Always

Steps to Reproduce:
1.Add replay gain tags.
2.Activate replay gain
3.Activate fadeout
4.Play quiet file
5.Play loud file

Actual Results:  
The volume gets adjusted after about a second.

Expected Results:  
The volume gets adjusted immediately.

Backend: gstreamer 0.10

This depends on the phonon backend in use, and as far as I can remember it is not possible to have both working at the same time. Reassigning anyway.

FWIW, since you are using Debian testing, you do not have the latest version of the phonon backend, please upgrade first.

This backend for Phonon is no longer maintained or supported, and has not been for quite some time. Please use the VLC backend instead--which is the recommended and maintained replacement--and see if you can reproduce the issue there. If you can, please open a new bug report at https://bugs.kde.org/enter_bug.cgi?product=phonon-backend-vlc.
