In the compose window is settings->spellchecker, which offers to set the default language. This setting is not persistent, effectively making the setting superfluous as it needs to be set again for each new email.
There is no equivalent setting in kmail->settings, perhaps that is the bug, and it's only ever being selectable for each composer window.
kmail-settings->configure kmail->composer only has a language setting for autocorrection. My default spell checker is different (English - New Zealand, which isn't available in the autocorrections drop-down).

Reproducible: Always

Steps to Reproduce:
1. Set KDE to British English, with spell checkers for New Zealand English.
2. Compose a new email.
3. In the compower window, setting->spellchecker: select English (New Zealand).
4. Save, send etc this email.

Actual Results:  
5. Compose a another email, the spell checker default language is back to English (UK).

Expected Results:  
When kmail is set to spellcheck new emails in English (New Zealand), it should not select another language.

normal it's specific at each composer.
you can specify it in identity.

And for  autocorrection it takes settings from kde so if there is not your language it's a kde problem not kmail.
