In kaddressbook with a 3-column view, the left column has an address book list. In RMB Folder properties the icon can be changed for each address book. The default is the "network server" icon. Changing the icon here and saving makes the address book name disappear from contacts displayed in the right column.
The only way to reset this is to go to system settings→personal info (select address book source)  →Modify→OK.

Steps to Reproduce:
1. Configure a new address book in system settings→personal info, use any empty diretory.
2. Create a contact in this.
3. RMB on the address bok name in column 1 of the 3 column view in kontact, Folder→Properties. Change icon.
4. Select this contact in column 2.
Actual Results:  
Column 3 no longer shows the name of the address book this contact came from.

Expected Results:  
The name of the source address book is shown with the contacts info.

I can not confirm this on a Kubuntu, KDE 4.12.3 system.
I tested it with the "Personal contacts" and "vCard-Folder" resources.

Is in the third column the hole entry missing: "Address Book <folder name>" or just the folder name?

It can be fixed.
