This is the example pdf file taht displays incorrectly
Can't confirm this. Version 0.15.5 (KDE 4.9.5) Gentoo Linux Everything worked well. Close it? With identical versions (Version 0.15.5 (KDE 4.9.5) ) on Kubuntu 12.10 it still doesn't work right. "Books Books Books" still does not display in okular. I don't see any "Books Books Books" if i open the file in acrobat reader, can you please post a screenshot? If you can provide the information requested in comment #4, please add it. Created attachment 77326 [details]

Okular saves the formular input data, even if you download a file again from the web. Please delete:
~/.kde4/share/apps/okular/docdata/255107.CertificateStreet_BL_135(2).pdf.xml (or similar)
The beaviour of my system is documented in the attached screenshots.
screenshot_1_orig.png: Initial load of your PDF
screenshot_2_form.png: Change the first input field to "Books Books Books"
screenshot_3_result.png: Hide the formular fields and you can see your input embedded in the document
Which version of poppler do you use. I have 0.20.5 as it is marked stabel in Gentoo.
I guess in Ubuntu it's 0.20.4.
Try a newer Version and file a bug at Ubuntu if this solves your problem
If I understand corrently: you are typing "books books books" in Okular, but it disappears when you click on Hide forms. It seems to work here, please reopen if I misunderstood and/or you can reproduce the issue with recent Okular and Poppler versions (eg poppler >= 0.24 and Okular >= 0.19.0 [ie KDE >= 4.13.0]).
