Randomly, at least with no pattern that I can determine. Close an application and it's taskbar entry remains. This remaining 'artifact' cannot be actioned. It goes though when you start a new application.

Reproducible: Sometimes

Steps to Reproduce:
Open/close applications
It happens randomly

Actual Results:  
Although the application is closed, it appears in the task bar

Expected Results:  
On closing an application it's taskbar entry should close/vanish.
