Is this an NVIDIA card?  Try using the latest NVIDIA drivers.

No, i have Mobile Intel® GM45 Express Chipset. 
After upgrading from 11.3 to 11.4 my system is unstable. Sometimes a have got few errors at once. Maybe another crash force this crash.
