Version:           4.6 (using KDE 4.6.0) 
OS:                Linux

Please look at the attached image. As you can see, I have an icon for cachegrind files. Yet it is not shown in dolphin (or folder views, etc). Is it a bug or am I doing something wrong. KDE 4.6 here

Is the file detected as the correct type? On your screen shot, the "Type" and "Contents" fields in the properties dialog are obscured. It might be the added number to the file extension that prevents correct type detection.

Yes, it is detected as the correct type. It opens with the correct program. I also can reproduce the same bug with other file type patterns (like *.kdb)
