Version:           0.9.5 (using 4.3.5 (KDE 4.3.5), Mandriva Linux release 2010.0 (Official) for i586)
Compiler:          gcc
OS:                Linux (i686) release 2.6.31.12-desktop-3mnb

Attempted to fill out passport application form (http://www.ppt.gc.ca/form/pdfs/pptc054.pdf) -- this failed for various reasons:

- Check boxes show double-quote rather than X (font available?)
- Place of Birth section's Prov/Ter/State field inoperative
- Page 1
  - credit card number field does not auto-insert spaces
  - authorization amount does not add ".00" to amount

Passport applications are usually submitted to organizations which are rather anal about the content, so I had to resort to firing up an old Windows box to prepare this application (Yuck!)

Hi,

I believe your third issue is a result of bug 177473 (Okular does not support Javascript right now).

It would be appreciated, I think, if next time you could file multiple reports instead of just one if you're having more than one issue.

Double-quote? Don't really understand but can't reproduce checkboxes showing an X - they seem to show a tick.
Please file a new, separate bug with further details if you can reproduce.

The rest I can confirm.
It is caused by Okular's very limited Javascript support, so I'm adding tracking by the JS meta-bug. Also, technically, this is a wishlist because Okular never claimed to support this.
