Hi, I believe your third issue is a result of bug 177473 (Okular does not support Javascript right now). It would be appreciated, I think, if next time you could file multiple reports instead of just one if you're having more than one issue. Don't really understand but can't reproduce checkboxes showing an X - they seem to show a tick. Please file a new, separate bug with further details if you can reproduce. The rest I can confirm. It is caused by Okular's very limited Javascript support, so I'm adding tracking by the JS meta-bug.
