When editing a form (see link to form below), text is not kept when going out of the form view. Checked check boxes appears though. This then also applies to printing of the filled in form.

Happens with both Okular and Evince, so this is a Poppler issues.
Please report a bug for the Poppler library, at https://bugs.freedesktop.org, "poppler" product, "general" component.
