lets see...  how to explain this problem so that it gets solved.

let me explain the steps here.
1.  Add programs to the plasma (Quick Launch in Windows)
2.  Reboot and the programs are properly displayed.
3.  Remove the programs "Remove this icon (Ctrl R) and then reboot again.
4.  The place where the icons were present now has a Icon with no application attached to it.
5.  Remove this icon again and reboot and after starting, the icon still remains there.
6.  Remove the icons and then delete the Plasma* files from the .kde/share/config location and only then the icons are gone.

This is a bug as its not able to write/update into the plasma files properly.

we did have a bug with not being able to remove plasmoids, but I'm pretty sure it was fixed...

4.2.1 was recently released, could you test with that please and tell us whether that fixes it?
