Version:           unknown (using 4.2.00 (KDE 4.2.0) "release 97", KDE:KDE4:Factory:Desktop / openSUSE_11.1)
Compiler:          gcc

Every start up new items are created next to my quicklaunch icons. The first time this problem occured, my actual items were replaced.
These 'white' icons are 0 byte in size.

This bug appeared when I updated from qt4.4 to qt4.5-RC

this bug has been resolved on my machine either with the KDE 4.2.1 packages or the final qt4.5 packages. Great Work

Maybe this should have also resolved other reported 'ghost items'? https://bugs.kde.org/show_bug.cgi?id=183635
