When kruler is vertical, the correct position indicator is used.
When kruler is horizontal, a shorter arrow as position indicator is displayed and a strange rectangular glitch is at the opposite side of the tip. I'm sorry, I can't provide a screenshot for a better explanation because mouse cursor is 
not saved on the snapshot.

which qt do you use ?
Perhaps it's a qt bug in windows.
Could you test with last qt4.5 please ?

I've updated to KDE 4.3 with qt 4.5.2 and the problem still exists.

P.S: I'm sorry for the delay with the answer but the computer with MS Windows is not mine.

Updated to KDE 4.10.2.
I can confirm the bug.
With East and West orientation the glitch still exists.

Here with kruler 5.0, KF5.21 I don't see any problem.
The last confirmation is from 2009. Maybe we can close it ?
I'm not sure the comment from Vincenzo is actually about the same issue. In several KDE 4.x release the icons used for the orientations in the context menu were wrong, maybe Vincenzo refers to that ?

Resolved with Aurélien's patches; we no longer (ab)use the cursor to show information.
